All pre-MAC J12 LMRP and LCD documents are located at the CMS website as part of the MCD (Medicare Coverage Database).  To help you locate these legacy documents, we have created complete indexes by region.

Each index lists only LCDs which were current/active at the time of the J12 MAC transition.  For previous versions or retired LCDs, please use the CMS Medicare Coverage Database Search.
